Information

You can apply for an Australian Criminal History Check (ICHC) using this application form.

• When you have submitted your application for the check, Fit2work will assess your application for completeness.
• When your application is considered complete, you will receive an email with an ICHC reference page that will include a check reference number.
• Please note: you do not need to provide the results report to Nursing Council of New Zealand. Your NPHC results will be sent directly to Nursing Council of New Zealand and you will also be emailed a copy by Fit2work.
